Purpose: to provide intake and coordinate services, which allows each person with a documented disability equal opportunity and accessibility to educational programs, college sponsored social events, facilities, and career opportunities at Yavapai College.
Philosophy: Yavapai College is committed to the development of personal growth, mutual respect for all cultures, and the concept of life-long learning for qualified individuals with disabilities, as mandated under Section 504 of the Rehabilitation Act (1973) and the Americans with Disabilities Act (ADA 1990).
Student Responsibilities: To receive accommodations/services you will need to do the following.
1. Contact the Disability Resources/ADA Coordinator Office six to eight weeks, or as soon as reasonably possible, before the requested accommodations are needed at the above phone number or e-mail address for an appointment.
2. Complete a Request for Services form and Release of Information form.
3. Provide recent documentation that provides diagnosis of a disability and it's functional impact to one or more major life activities. Any documentation provided is confidential and used for the sole purpose of determining which accommodations are appropriate for your individual situation.
Faculty Responsibilities:
1. Maintain admission and academic standards
2. Identify and assist in the removal of attitudinal, architectural, and informational barriers.
3. Refer students with disabilities and suspected disabilities to the Disability Resources/ADA Coordinator Office.
4. Provide reasonable accommodations to students with verified disabilities. Faculty are NOT required to provide accommodations to students who have not self-identified the nature of their disability and requested accommodations through the Disability Resources/ADA Coordinator Office.
